Admitted Student Forms & Info
We can’t wait to see you in class!
On or before the first day of school, your child’s folder in the preschool office, should contain the following completed forms: Emergency Contact, Authorization for Pick-Up, Quick Contact Card, VA School Health Form, and Preschool Agreements.
Please print and fill out the five admitted student forms below and bring them with you on orientation day. We will also need a copy of your child’s birth certificate.
